Centrelink Consent

Concession and /or Government rebate validation section

This consent will be used for the sole purpose of authorising Centerlink to provide information to Origin to assess your eligibility in relation to concessions or services provided by Origin:

• I authorise Centrelink to confirm with Origin the current status of my Commonwealth Benefit and other details as they pertain to my concessional entitlement.  This involves electronically matching details I have provided to Origin with Centerlink or  Department of Veterans’ Affairs (DVA) records to confirm whether or not I am currently receiving a Centerlink or DVA benefit.
  
• I understand that this consent, once signed is effective only for the period I am a customer of Origin.
  
• I understand that this consent, which is ongoing, can be revoked any time by giving notice to Origin by calling 131 002
  
• I understand that if I withdraw my consent, I may not be eligible for the concession provided by Origin.
  

Centrepay deduction Section (if applicable)

If you are currently paying your electricity bills through Centerpay, please complete this section:

• I authorise Centerlink to deduct the amount I have previously nominated (and is currently being deducted) from my fortnightly Centerlink payments even if my payment type changes in the future and pay it directly to Origin.
  
• I understand that the deduction is voluntary to meet my electricity payment and that I can vary, suspend or cancel the deduction at any time by contacting Centerlink.
  
• I authorise Centerlink, Origin and its agents and contractors disclosing my personal information to each other for the purpose of checking my account number, the amount I wish to pay and to reconcile my payment deduction details, or as part of the transfer of data to Origin as part of its purchase of the Integral Energy retail business.
